IMPORTANT NOTE
Team official on application form MUST complete and fax this form to our office @ (905) 669-1269 by August 13th, 2008, otherwise coaches passes will NOT be issued to your team.  Additionally, Coaches Passes will NOT be issued until team rosters & hotel information is submitted.  Hotel accommodations must be booked through tournament office.  This is a prerequisite of acceptance in the tournament.

Coaches Passes will NOT be available at the arenas once the tournament has commenced and will only be issued in advance of the tournament once the required form, roster & hotel information is submitted.
